Our Mission

To empower people with knowledge so they can make the best decisions in regards to “What’s Next” in their healthcare journey. This site was developed as a way to educate people to be an advocate for themselves and their family members.

About Us

In our careers, we have spent countless hours meeting with patients and families to explain “What’s Next” in their healthcare journeys. Jessica has worked as a nurse liaison for all types of post acute care. It is her role to review clinical records and identify the medical conditions that make people appropriate for a particular level of care. Geoffrey has a Master’s in Education and works in home health care. It is his role to set up patients with home health, educate them on what to expect, and obtain all the necessary documentation. We both educate patients, families, social workers, physicians, and insurance companies on the the next levels of care.

Our Journey

The idea for Healthcare Roadmap began when Jessica received a call from a distraught daughter, wondering why hospital staff continually recommended one option for her mother’s care, despite her “thinking” there were other options. After a brief conversation, and newfound knowledge based on medical criteria, the daughter was then able to discuss further options of care with the physician and hospital staff.

This was, by no means, an isolated incident. We frequently receive phone calls from families inquiring what services are available for their loved one currently in the hospital or suffering a decline at home. Often, different levels of care are not thoroughly explained to families. People are simply provided a list of one type of post acute care option and told to choose a facility, with little more explanation or alternatives. Families have called when they felt their loved one required “more care” than what was being offered. I often find myself explaining the benefits of not only the level of care of where I work, but also educating people on all levels of care and the benefits of each, based on their individual situation. These experiences made me realize this information needed to be more readily accessible to the general public.

What began as an idea to educate the public based on work experiences has proven useful on a personal level as well. I realized just how valuable the information on the site was when I was making the final revisions to the hospice criteria page. As I was reading through the criteria, I discovered I was explaining my grandmother’s condition with Alzheimer’s Disease. Although I had worked in hospice for almost 10 years, it took reading the information on the computer before it clicked. I couldn’t believe I hadn’t thought of getting hospice involved sooner. If the website could educate and help our families, I knew millions of other people would benefit from the information as well.
