The purpose of information on this Site is to educate and connect consumers with healthcare services for loved ones. When you complete the Make a Referral portion of our Site, you will be connected with various healthcare facilities. Healthcare facilities can pay to receive promotional benefits with our Site. Nothing on this website is intended to be used for medical diagnosis or treatment. The information should not be considered complete or relied on to suggest a course of treatment for a patient. It should not be used in place of a visit, call, consultation, or the advice of your physician or other qualified health care provider. Although we do our best to protect your Personal Information, we cannot guarantee the security of your Personal Information transmitted to our website. Any transmission of Personal Information is at your own risk. We are not responsible for circumvention of any privacy settings or security measures contained on the website.

 

International Use

Although this site may be accessible worldwide, we make no representation that materials on this site are appropriate or available for use in locations outside the United States, and accessing them from territories where their contents are illegal is prohibited. Governing Law

These Terms are entered into in the State of Illinois and shall be governed by and construed in accordance with the laws of the State of Illinois, exclusive of its choice of law rules. Each party to these Terms submits to the exclusive jurisdiction of the state and federal courts sitting in the County of Winnebago in the State of Illinois, and waives any jurisdictional, venue, or inconvenient forum objections to such courts. In any action to enforce these Terms, the prevailing party will be entitled to costs and attorney’s fees. These Terms constitute the entire agreement between Healthcare Roadmap and you pertaining to the subject matter of this Agreement.
